Enrollment Forms

Welcome to KiddiTime Family Childcare! We are excited to be a part of your child’s early learning experiences!

Once your application is approved, please read/complete/sign all required enrollment forms and send them to us via email or in person. There is a non-refundable enrollment fee of $50 per child or $75 per family. We need to review and confirm all required forms for each child before they can attend KiddiTime Family Childcare.

We want you and your child to be pleased and comfortable with your decision to join KiddiTime. Therefore, we offer a 2-week trial period, in which parents and providers reserve the right to terminate the contract agreement without advance notice.

  2. Immunization record (complete/sign/date)

  3. Permission to Administer: LIC 9221 (complete/sign/date)

  4. Consent for Emergency Medical Treatment: LIC 627 (complete/sign/date)

  5. Consent for Nebulizer Care: LIC 9166 (complete/sign/date)

  6. Photo/Video Release Form (sign/date)

  7. Identification And Emergency Information: LIC 700 (complete/sign/date)

  8. Notification of Parent’s Rights: LIC 995A (sign/date/keep a copy)

  9. Parent Notification Additional Children In Care: LIC 9150 (sign/date)

  10. Caregiver Background Check Process: LIC 995E (read/keep a copy)

  11. Family Child Care Consumer Awareness Information: LIC 9212 (read/keep a copy)

  12. Sunscreen Permission Form (sign/date)
