Enrollment Forms
Welcome to KiddiTime Family Childcare! We are excited to be a part of your child’s early learning experiences!
To start the enrollment process, please submit an application for each child. Once your application is approved, we will send you additional information including all the required enrollment forms below. Please review, complete, and send all forms back to us via email or schedule a time to drop them off.
There is a non-refundable enrollment fee of $75 per child or $125 per family. We need to review and confirm all required forms for each child before they can attend KiddiTime Family Childcare.
We want you and your child to be pleased and comfortable with your decision to join KiddiTime. Therefore, we offer a 2-week trial period, in which parents and providers reserve the right to terminate the contract agreement without advance notice.

Notification of Parent’s Rights: LIC 995A (turn in/keep a copy)
Parent Notification Additional Children In Care: LIC 9150 (turn in/keep a copy)
Caregiver Background Check Process: LIC 995E (read/keep a copy)
Family Child Care Consumer Awareness Information: LIC 9212 (read/keep a copy)
Consent for Nebulizer Care: LIC 9166 (turn in if needed)
Permission to Administer: LIC 9221 (turn in if needed)
Epi-Pen Pre-Authorization (turn in if needed)
